You are looking at one on the rarest religious medals of Theotocos - the Unfading Bloom. It depicts the Blessed Virgin holding her divine son and a lily flower.
The inscription on the front side stands for the title of the icon - "Unfading Flower". The prayer text on the back side says: "Rejoice, O Bride of God, rejoice, mystical rod which blossomed the Unfading Flower; rejoice, O Sovereign Lady, through whom we are filled with joy and inherit life".
